Hours will be Monday- Friday ( 9 AM to 5 PM) Full Time or Part Time. ( Flexible Hours)

Primary Responsibilities:

• Making outbound calls to assess members' current health status
• Identifying gaps or barriers in care
• Reinforce self care-management
• Making referrals to outside sources
• Coordinating services as needed
• Encouraging members to make healthy lifestyle changes
• Documenting and tracking findings

You’ll need :

• Strong computer and software navigation skills
• Great Phone skills, friendly voice

Job Types: Full-time, Part-time

Pay: $15.00 - $20.00 per hour
