As a Behavioral Health Navigator, you will be part of the Access Team and will be responsible for reaching out to consumers regarding satisfaction with past appointments as well as availability for upcoming appointments. You will increase engagement in appointments by sending reminders and offering assistance with transportation when possible. You will also rotate between catchment area emergency departments and clinics, including the UnityPoint Behavioral Health Urgent Care, as well as catchment area hospital inpatient medical and behavioral health floors. You will be available to existing consumers to ensure continuity of care with current services and available to new consumers to perform intake assessments and connection to services.

Instead of the above, a minimum of two years of relevant work experience. This is a remote position; preferred candidates live within the state of Wisconsin.
Our Care Partners (Certified Caregivers) provide direct care to our senior residents following a personalized treatment plan. High School Diploma or equivalent.
Candidate should enjoy coaching and managing a team of 50-75 in a high-volume setting. Two consecutive days off per week.
