Are you a passionate clinical pharmacist looking for a dynamic and challenging role in the healthcare industry? Look no further! In-Clinic Rx is seeking a highly skilled Clinical Pharmacist to join our team and take the lead in providing exceptional Chronic Care Management (CCM), Transition Care Management (TCM), and Remote Patient Monitoring (RPM) services through a Collaborative Practice Agreement (CPA) with independent medical facilities in the Lumberton, NC area.
As the Clinical Pharmacist, you will be responsible for executing these clinical service programs, including monitoring reimbursement rates, enrolling new patients, and managing RPM devices. Your responsibilities may expand or vary, depending on the unique needs of each independent practice, providing you with a diverse and exciting workload.
In this role, you will have the opportunity to:
  • Spearhead patient enrollment, device management, and data interpretation for RPM services requested by medical facilities.
  • Communicate critical alerts to providers promptly.
  • Implement drug therapy changes as allowed by the CPA with the provider.
  • Participate and lead topic discussions as scheduled in the In-Clinic Rx PGY-1 Community Pharmacy Residency Program.
  • Precept PGY-1 Community Pharmacy Residents.
  • Participate in In-Clinic Rx Fellowship Program.
  • Provide data to providers regarding critical alerts promptly.
  • Establish and maintain professional relationships with all team members within the medical practice.
  • Provide recommendations or implement therapy changes, when necessary.

To be considered for this role, you must have:
  • A graduate degree from an ACPE-accredited school of pharmacy or international equivalent.
  • A current, valid license to practice pharmacy in North Carolina, South Carolina, Georgia, and Virginia.
  • Completion of APhA/ASHP Accredited PGY-1 or PGY-2 Residency Program.
  • Current CPR certification.
  • Certification for Immunization.
