Essential Functions:

  • Ability to understand and assist two or more providers with clinical care.
  • Coordinate consult/referral, hospital/ER, community resource follow-up/tracking process for the practice.
  • Serves as an advocate for patients and utilizes appropriate resources.
  • Works collaboratively with provider(s) and other staff to ensure the delivery of quality care to patients to ensure best patient outcome.
  • Pre-visit planning workflow to ensure care completion prior to visit whenever possible.
  • Review after visit summary with patients whenever appropriate.
  • Involving the patients in activities to improve their health (patient engagement).
  • Educating the patient about self-management tasks they can undertake to gain greater control of their health status.
  • Manage many aspects of the patient’s care: referrals to specialists, hospitalizations, ER visits, ancillary testing, specialty medications and other outside services.
  • Responsible for being available to provide telephone advice per protocol, handle urgent calls and emergent calls.
  • Assess barriers when a patient has not met treatment goals, is not following a treatment plan of care, or has not kept important appointments.
  • Serve as a liaison for out of state patients.

Qualifications:

  • Certified Medical Assistant in the State of California required.
  • High School Diploma or equivalent required.
  • 1-2 years Medical Assistant experience required.
  • Experience in Allergy/Immunology and Pediatrics preferred.
  • Applicable procedure training (PFT, ENO, etc.).
  • MS Office Suite preferred.
  • EMR experience preferred.
  • Customer service experience required.
  • Team mentality and positive working attitude.
  • Strong verbal/written communication skills.
